At Peace Hospice Care Services

At Peace Hospice Care’s plan always involves patient and family choices. At the heart of the hospice philosophy is the belief that patients can remain independent and in control of their lives. We involve patients and their loved ones in every decision about their care, encouraging them to fully participate.

At Peace Hospice Care’s services include an interdisciplinary group of professionals and volunteers who will develop with each patient and family/caregiver a plan of care, which will include, as appropriate, the following services:

  • Physician
  • Registered nurse
  • Medical social worker
  • Counseling
  • Spiritual support
  • Therapists (physical, occupational, and speech)
  • Dietary counseling
  • Certified Nursing Assistants
  • Homemaker services
  • Hospice volunteers
  • Consultant pharmacist
  • Durable medical equipment related to the hospice patient's terminal diagnosis to include hospital beds, oxygen and more
  • Medications and medical supplies related to the hospice patient's terminal diagnosis for comfort and pain relief
  • Inpatient care: short-term stay for symptom control and planned respite for family/caregiver
  • Bereavement support and follow-up for at least 13 months after death for loved ones, family and caregivers
  • Training and support for the care of the patient
  • 24-hours-a-day access for care and consultation

Anyone may make a hospice referral. Hospice care is not medical-condition specific. At Peace Hospice Care will not deny service based on inability to pay or discriminate based on age, race, gender, religion or otherwise. Financial coverage is available through the Medicare hospice benefit, Medicaid or private insurance.
